Community Paediatrics Specialist medical provision for children who are referred in with a range of conditions physical conditions which include Physical disability Learning disability General conditions The service also provides medical and nursing assessments to Looked after Children Children where abuse has been alleged / occurred Access to the service is via referral
COAST An acute community children’s nursing team Avoid a Hospital admission Or help get a child home more quickly from hospital 7 days a week The GP can refer a child if Ports City, Fareham & Gosport or East Hants GP One of 10 conditions, eg. asthma, gastroenteritis, chest infection Less ‘quality’’/general follow up referrals, more avoided admission referrals
Children and Young Person’s Therapies Provides an integrated therapy service: Occupational Therapy Physiotherapy Speech and Language Therapy Accept referrals of children who: Are aged 0 – 19 years Have a Hampshire, Southampton or Portsmouth GP or who attend a Hampshire, Southampton or Portsmouth school. Present with a complex or uneven profile of development Children whose needs cannot be met by those who work closely with them Working with Parents – the experts in their children’s care. Working in homes, schools, clinics – wherever the child is.
Muscular Skeletal Services (MSK) – Physiotherapy and Pain Our patients are adults over 18 Receive referrals of more complex patients from Southern Health’s physio service. Treatments include: Physiotherapy Joint injections Individual and Group treatments Service can refer onto QA for surgery Pain service For persistent pain (typically over 3 months), the service supports through group sessions to support self management Based at Crofton Centre, Stubbington
Podiatry Foot and Ankle conditions Sometimes Diabetes linked Foot Ulcers Nail surgery requirements Provided at various locations across Hampshire and Portsmouth Domiciliary service available for housebound patients. Close working with QA for acute issues Service does not cover routine nail care which is not provided by the NHS, but is available as a paid for service
Special Care Dental Service Our patients are children and adults who have particular needs: a physical, sensory, intellectual, mental, medical, emotional or social impairment or disability or a combination of these which may prevent them from accessing care through the General Dental Services Specialist occasional care, supporting care in general dental practice by providing a single or staged course of treatment. General Anaesthetic services from Poswillo on the QA site Dental surgery services from 18 locations across Hampshire. Domiciliary care services in people’s homes and nursing homes.
Sexual Health Services Drop in and appointments at various locations Contraception from condoms to vasectomies Testing and treatment for Sexually Transmitted infections. Population screening – eg. Chlamydia screening Psycho Sexual Counselling Treetops – Sexual Assault Referral Centre Work with people of all ages
Substance Misuse Services Drug and Alcohol services in various locations. Drop in and booked sessions. Medics, nurses, counsellors, recovery workers, peer workers. Recovery focused Programmes Inpatient Detox centre at Baytrees, St. James, Portsmouth
