1.link to the website of Guidance Section, Office of Student Affairs 2.click “Student Learning Portfolio” Student Learning Portfolio

4.fill in your personal data Student Personal Data Student ID number Name Department Address Cell Phone submit Please verify the data(Address and cell phone must be filled in.)

5.click the button of “confirm”, then fill in the leave data Notice : Please make sure the data is correct. After click the button of “submit”, the data can’t be cancelled or revised. Student Leave Data Student ID number Academic year Semester The date of the beginning The date of the end The types of the leave The reason For general reason For examination For registration Submit Cancel

7.enter the class schedule screen, choose the class Period Time/Date Choose the class you want to apply for the leave

Please don’t tick off the class which you don’t need to apply for leave. Notice : Please make sure the data is correct. After click the button of “submit”, the data can’t be cancelled or revised.

8.after click the button of “submit”, the screen will show the leave form. Please print it out. Notice : Please install the Acrobat (Computer software) and print the leave form by laser printer for the definition of the bar code.

1. After printing the leave form, please take it to the teacher, the tutor, chairman or dean, then take the leave form to CL If the online application is failed, please link to the website of Guidance Section and download the blank leave form. Special Notice
