Diabetes Medical Management Plan (DMMP) O Basis for all school-based diabetes care plans O Developed by: -student’s personal health care team -parent/guardian O Signed by a physician or Advanced Practice Registered Nurse (APRN) O Individualized to student O Coordinated collaboratively by the school diabetes team: -school nurse -health coordinator -school administrator -other school personnel (Delegatees) `-parent/guardian -the student KBN 2014
