2014 PPE Disclosure Statement It is the policy of the Oregon Hospice Association to insure balance, independence, objectivity, and scientific rigor in all its educational programs. All faculty participating in any Oregon Hospice Association program is expected to disclose to the program audience any real or apparent affiliation(s) that may have a direct bearing on the subject matter of the continuing education program. This pertains to relationships with pharmaceutical companies, biomedical device manufacturers, or other corporations whose products or services are related to the subject matter of the presentation topic. The intent of this policy is not to prevent a speaker from making a presentation. It is merely intended that any relationships should be identified openly so that the listeners may form their own judgments about the presentation with the full disclosure of the facts. This presenter has no significant relationships with companies relevant to this presentation to disclose. 1

Honor Rate What does success look like? Average deaths in hospice each year: 10,500 (From Oregon Hospice Association Summary Reports ) Estimated # with a donor designation: 7,770 (based on 74% donor designation share in Oregon) Estimated # eligible to donate each year: 3,885 (historically approximately 50% of hospice patients meet donation eligibility guidelines to donate corneas and tissues) Maximum Honor Rate for Hospice = ~3,885/ year

Implementation Options There are several models we could follow for starting a referral policy Information in Admit Packet Imminent Expiration Referrals Referral at Expiration Lions VisionGift follows different protocols to meet the needs of each hospice partner Lions VisionGift is the referral organization for all eye and tissue referrals

Potential Referral Flow Patient expires and your staff calls referral line LVG staff will call back to assess donor potential Only if eligible, will we approach the family (LVG asks you to get contact information for the decision maker) If the family chooses donation, LVG needs to coordinate with the funeral home the family is choosing
