For Agent Use Only – Not for public distribution How The Benefit Is Paid Paid in a lump sum per category directly to insured. Multiple critical illnesses may be paid within a category as long as the maximum benefit for that category is not exceeded. You are eligible for benefits from another category if the date of diagnosis is 180 days from the previous date of diagnosis for a covered Critical Illness. If you should have two or more Critical Illnesses diagnosed on the same day, Cash Advantage will pay the larger of the Critical Illness benefits for the conditions diagnosed.

SAM Two Years Later: Sam undergoes Coronary Artery Bypass Surgery. Policy pays Sam $2,500 (25%) from Category Two: Heart/Stroke Conditions. Seven Months After Surgery: Sam has a Heart Attack. Policy pays Sam $7,500 (75%) from the remaining balance of Category Two: Heart/Stroke Conditions.

Hospital Indemnity Benefit –Daily cash benefit for hospital confinement as an inpatient with maximum of 30 days. –Pays 2 times the amount if in Intensive Care Unit –Daily benefit amounts between $100 - $1000 (in $100 increments) Rider availability varies by state
