AF4Q is the signature effort of the Robert Wood Johnson Foundation Lift the overall quality of health care in targeted communities Reduce racial and ethnic disparities Provide models for national reform AF4Q asks the people who get care, give care and pay for care to work together toward common, fundamental objectives that lead to better care. Aligning Forces for Quality (AF4Q)

Rising to the challenge Launched and reported on the nation’s first comprehensive, statewide patient experience survey Nurtured the development of the national Health Care Home movement Supported the DIAMOND program, which has become a national model for treating depression through collaborative care Saved an estimated $234 million through the High-Tech Diagnostic Imaging (HTDI) project Developed the nation’s first Total Cost of Care measure Helped avoid nearly 8,000 hospital readmissions through the Reducing Avoidable Readmissions Effectively (RARE) Campaign
