About Us “ To provide leadership for a collaborative service and resource network that supports individual choice, independence and dignity for older and disabled adults.”

What is an ADRN? Streamline access to long-term supports & services Designed to enhance access to community living, personal choice & independence. Collaborative effort by partners who are either resources or referral sources for older and disabled adults Part of a nationwide effort to restructure services & supports for: Older adults All persons with disabilities Family members Care providers

What is an ADRN? AoA & CMS share the vision of ADRN’s becoming integral components of health and long-term care reform & the development of effectively managed person-centered service systems at national & state levels.

What is an ADRN? ADRN’s are not located in a single physical place ADRN’s & their functions are not necessarily carried out by a single agency ADRN’s are about networks of state & local organizations working together to provide consumers with integrated access points to long- term care services & supports ADRN’s have multiple points of entry with streamlined access to services

Key Facts  76% of ADRN’s have online Medicaid applications available  36% have online decision making tools  29 states maintain publicly-accessible databases of information & resources across the state  13 states are developing similar statewide capacity  ADRN’s report high levels of satisfaction—an average of 91% satisfaction with services
