North Orange County ROP Scantron Registration Procedures

Instructions for Students to Complete the Enrollment Form Please follow the directions as the instructor goes through them. Please do not get ahead of the instructor. It is easier to complete the enrollment form correctly the first time. We will be filling out the boxes at the top of the columns first. When we complete filling in the boxes you can then go back and fill in the bubbles. All entries will be left justified, starting in the first box on the left.

1.Student Name Print LAST name. Enter a comma. Skip a space. Print FIRST name. Skip a space. Print MIDDLE INITIAL.

2. Student ID Number SKIP THIS

3. Gender Bubble only one. –Female or Male

4. Social Security Number SKIP

5. Ethnicity Bubble only one. If you have a mixed heritage, choose the one you most identify with. If your ethnicity is not listed, bubble in other and write your ethnicity on the line at the bottom.

6. Native Language Bubble only one. The language spoken at home

7. Birth Date Bubble in the month of your birth. In the boxes at the top: –Print the day you were born. –Print the last two digits of the year you were born.

8. Primary Reason for Enrollment Bubble only one.

9. Other Status SKIP THIS.

10. Years of School Completed Print the last completed year of school. If you are in the grade 11, print 10.

11. Highest Degree Earned Bubble in the highest degree you have earned. High School students will bubble in None.

12. Are You Currently Enrolled in H.S.? Bubble yes if you are currently enrolled in high school.

12. Current Grade Level Bubble your current grade level.

12. Home High School Print your three digit home high school code. You can get the home high school code from the following pages.

14. District Code Bubble in the district code 99 for ROP.

15. Section Number Start on the left of the box. Print the section number of the class, Ex.: 51571

16. Course Number Start on the left of the box. Print the course number of the class. The course number always ends in a “0” Ex.: 51570

17 – 22 Other Sections & Courses Fill in other section and course numbers if you are enrolling in more than one section.

23. Today’s Date This date corresponds to the first day of attendance for this class. Bubble in the month. Print today’s date. Print the last two digits of the year.

23. Course Name Print the course name on the line next to the corresponding number. Ex: Medical Core

Course Title In the top right corner of the form there are two lines for the Course Title. –On the top line enter the course number. –On the bottom line enter the course name. –Same as course name

Student Name In the bottom right corner of the form there is a line for your name. Print last name, first name, and middle initial.

26. Mailing Address Print your street number. Skip a space. Print your street name. Skip a space. Print your apartment number if necessary.

27. Zip Code Print your 5 or 9 digit zip code.

28. Family Size High school students SKIP THIS

29. Disability / 30. Programs SKIP BOTH

31. Home Telephone Number Print your home area code and telephone number.

32. Work Number High School Students –Print your parents’ work area code and telephone number.

33. Work Extension High School Students –Print your parents’ work extension.

34. Emergency Phone Number If your emergency phone number is different from your home or work numbers: –Print your emergency area code and telephone number.

Bubble in the rest of the form Go back and bubble in the front and back of the enrollment form. Take your time and fill in the bubbles completely or you will need to redo them. When you are finished, bring the completed form to the instructor. Wait while the instructor checks your form. Return any borrowed pencils.
