“The ACSS team serves primarily individuals who are experiencing a major mental illness requiring longer term case management in the community to achieve and maintain essential life necessities such as adequate housing, nutrition, financial resources, medical and psychosocial care.”

 Self refer – call Centre or come into office  Physician or hospital referral  Voluntary service unless client is certified under the Mental Health Act  Referral is screened for client eligibility and to ensure that we are the appropriate agency to meet their request/needs

 Multi-disciplinary team consisting of three Registered Psychiatric Nurses, three Registered Social Workers, and two Health Care Workers  Ministry of Social Development and Social Innovation Mental Health Liaison  Case management involves providing mental health specific services through our office plus ensuring clients are connected with appropriate resources in the community

 Examples might include psycho education concerning diagnosis and medications, administering of injectable medications, skills teaching to help deal with symptoms, supportive counselling, access to psychiatrist for assessment and treatment if required  Advocating, liaising, referring, coordinating linkages with/for stable housing, financial resources, probation, addiction resources, doctors and other health care providers, social supports, vocational resources

Case management attempts to address the social determinants of health, supporting clients with achieving their goals of living stable, healthy, fulfilling lives as defined by the client.
