2 If you have a HealthPartners username and password, enter it here with your company name and that is your only step to take your health assessment.

3 If you click register for an account on the first screenshot, you will see this screen next and need to enter your company name to continue.

4 Then you will need to verify if you have HealthPartners insurance.

5 If you click yes that you have HealthPartners insurance, this screen is step 1 of the registering process.

6 If you click yes that you have HealthPartners insurance, this screen is step two to the registering process.

7 If you click no to having HealthPartners insurance, this screen is step 1 of the registering process.

8 If you click no to having HealthPartners insurance, this screen is step two to the registering process.

9 Finally, new registrants must accept the conditions to continue.

10 Along the way, if you enter an incorrect company name, you will be prompted to call customer service.

11 If you try to log on prior to the program starting, you will see this screen.
