Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D900023) General Scope of Work  Full bridge replacement (substructures and superstructures) of the following bridges:  BIN – I-81 SB over Route 80  BIN – I-81 NB over Route 80  Removal of existing substructures and superstructures, including potential remediation and disposal of lead paint and asbestos- containing materials  Approach roadway reconstruction as required to connect the existing roadways to the new bridges, including guide rail replacement  Construction of asphalt paths on the north and south sides of Route 80 between the Route 11 intersection and the Route 11A intersection 9

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D ) General Scope of Work (Continued)  Cleaning of existing ditches along both sides of Route 80 between the Route 11 intersection and the Route 11 A intersection  Replacement of ground-mounted signs along Route 80 between the Route 11intersection and the Route 11A intersection  Coordination of utility relocation and/or preservation of existing utilities with utility owners  Development of a Work Zone Traffic Control Plan  Any additional Environmental and Construction Related Permits by Design-Builder 10

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D ) Right-of-Way  The Department owns all Right-of-Way for the existing structures.  No additional Right-of-Way acquisitions or temporary easements are anticipated to be required for this project.  Any Right-of-Way fencing damaged or removed by the Design-Builder during construction shall be replaced with new Right-of-Way fencing meeting current Department standards. 11

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D ) Utility Coordination – Existing Utilities In accordance with Part 4 – Utility Requirements, Design-Builder is responsible for:  Verifying utility facility locations  Identifying & resolving any conflicts not yet documented  Coordinating with utility owners for utility relocations  Providing stakeout and final layout of utility relocations  Protecting existing utilities  Cost of repairing damages to utilities caused by Design- Builder  Informing the Department of all meetings and correspondence with utility owners 12

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D900023) Utility Coordination – Existing Utilities  Known utilities located in the Project area:  National Grid 6-Inch gas line 18’+/- south of Route 80 centerline Primary aerial lines on the north side of Route 80 that cross over Interstate 81  Verizon of NY Duct bank 28’+/- north of Route 80 centerline  Time Warner Aerial lines on the north side of Route 80 attached to the National Grid poles - These cross under Interstate 81 and are attached to the underside of the Interstate bridges. 13

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D900023) Utility Coordination – Existing Utilities  Time Warner Cable shall be responsible for any necessary relocation of its aerial facilities within the project limit. The Design-Builder shall be responsible for negotiating a three-party agreement between the utility, the Department and the Design-Builder.  Impact on other utilities is dependent on the approach taken by the Design-Builder.  A meeting will be scheduled between the utilities and the short listed firms. The Department will invite the short listed firms and the utilities within the project limits to this meeting within the next two weeks. 14

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D900023) Contract Information  Project Completion Date shall be the earlier of:  October 31, 2016 or  The number of days past NTP proposed by the successful Proposer on Form SCD (ITP Appendix E)  Special Provision 6 -  Liquidated Damages will be imposed for not meeting the Project completion date and for days in excess of the number of calendar days indicated to complete the Milestones shown on Form SCD.  Up to 3 Early Completion Bonuses will be awarded: For the number of calendar days that all traffic in permanently transferred onto each new structure earlier than the respective Interim Completion Milestone Dates (with no further lane closures) and/or For the number of calendar days that Project Completion is achieved earlier than the Project Completion Date 15

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D900023) Quality Control (QC)  QC is Design-Builder’s Responsibility.  Design-Builder shall appoint a QC Manager responsible for the QC Plans for both Design and Construction.  Design QC shall be performed by the Designer in accordance with the QC Plan.  Construction Inspection shall be performed by an Independent Engineering Firm hired by the Design-Builder.  Materials Testing shall be performed by an Independent Materials Testing Firm or Laboratory hired by the Design-Builder.  The Materials Testing Firm shall report to the Construction Inspection Firm.  Offsite Plant Quality Control shall be performed by the Department. 20

Interstate 81 Bridges Over Route 80 in Region 3 Design-Build Project (PIN ; D900023) Quality Assurance (QA)  Design and Construction QA is the Department’s Responsibility.  The Department may perform QA on any of the Design- Builder’s Design and Construction activities.  The Department will utilize an Independent Consultant to perform QA of the Design-Builder’s Designs.  Construction QA will be performed by Department staff.  Offsite Plant Quality Assurance will be performed by the Department. 21
