Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) General Scope of Work (continued) Full Bridge Replacement of seven bridges in Ulster County (BINs 1025150, 1019680, 1040990, 1025480, 1026650, 1041150 & 1027090) Removal of the existing substructures and superstructures; Replacement of the existing bridges including new foundations, substructures, superstructures and approach slabs; Reconstruction of approach roadways as necessary to connect existing roadways to the new bridges; Development of a WZTC plan for each site; Coordination with Utility Owners and/or preservation of existing utilities

Fee Acquisition and/or Temporary Easements have been acquired for: BIN 1025150 (Route 42 over Bushnellsville Creek) BIN 1040990 (Route 212 over Beaver Kill) BIN 1025480 (Route 54 over Mara Kill) BIN 1026650 (Route 52 over Veerkeeder Kill) BIN 1041150 (Route 213 over Rondout Creek) BIN 1027090 (Route 55 over Rondout Creek) Acquisition maps are provided in Part 7 – Engineering Data No additional Right-of-Way acquisitions or temporary easements are anticipated to be required for this Project

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) Work Zone Traffic Control (WZTC) Requirements / Restrictions Alternating one-lane, two way traffic may be utilized at all Project sites except BIN 1041150 (Route 213 over Rondout Creek) The maximum length of any off-site detour, as measured from one approach end to the bridge along the detour route to the other approach end of the bridge, shall be 20 miles In lieu of an approved ATC, any off-site detour shall be routed over state highways only. If an otherwise acceptable ATC proposes an off-site detour over roadways other than state highways, the Department will most likely issue a conditional approval requiring documentation of a commitment from the agency or agencies with jurisdiction to allow the use of their roads for the detour. Such documentation shall be provided no later than July 1, 2015 No off-site detours or lane reductions shall be permitted at any Project site between November 15 and March 1, except at BIN 1041150 where the restricted dates are July 1 – January 2.

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) Utility Coordination – Existing Utilities In accordance with Part 4 – Utility Requirements, Design- Builder is responsible for: Verifying utility facility locations; Identifying & resolving any conflicts not yet documented; Coordinating with utility owners for utility relocations; Providing stakeout and final layout of utility relocations; Protecting existing utilities; Cost to repair damages to utilities caused by Design-Builder; Informing the Department of all meetings/correspondence with utility owners.

Any necessary relocation work will be performed by the affected utility owner. Refer to Part 4, Appendix A for specific utility companies present at each Project site and approximate relocation time frames.

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) Utility Coordination – Existing Utilities – Relocations by DB BIN 1041150 – Town of Marbletown, Route 213 over Rondout Creek Verizon The Design-Builder shall install nine (9) replacement conduits on the new structure, including all hangars, rollers and other appurtenances. It is currently anticipated that Verizon shall supply the Design-Builder with the conduits, rollers and all other appurtenances necessary for installation of the conduits Ulster County Install 16” diameter steel abutment sleeves and roadway sleeves to accommodate the future installation of an 8” water main across the bridge and diagonally across Route 213 west of the structure Sleeves, including end caps, will be provided to the Design-Builder by Ulster County

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) Utility Coordination – Existing Utilities – Relocations by DB BIN 1027090 – Town of Wawarsing, Route 55 over Rondout Creek Town of Wawarsing – (Water) Design-Builder shall be responsible for: Installation of a 12” diameter water main along the east side of the bridge, including replacement of any hydrants or valves impacted by the project; Removal of the existing 12” diameter waster main; Any necessary temporary relocation(s) of the water main to maintain service during construction; and Obtaining all necessary approvals from the Department of Health The Town of Wawarsing shall requires two (2) weeks to review the planning, design and construction documents

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) Utility Coordination Three Way Coordination – Design-Builder, Utility Company & State Coordination meeting will be held between the Department, all short-list firms, and the utility companies shortly after issuance of Final RFP. DB Responsibilities The Design-Builder is expected to coordinate with any and all affected utility owners and the Department to negotiate and execute 3-party Final Utility Work Agreements. The Design-Builder is responsible for participating in the negotiations & coordinating the work schedules. Status of Preliminary Utility Work Agreements Signed Preliminary Utility Work Agreements are anticipated to be included in the Final RFP

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) Contract Information Lump Sum Design-Build Contract; Best Value Selection Process (50% Price, 50% Quality); Project Completion Date shall be the earlier of: October 1, 2017; or The number of days past NTP proposed by the successful Proposer on Form SCD (ITP Appendix E) Liquidated Damages will be imposed for not meeting the Project Completion date and for Impacts to Traffic in excess of the number of calendar days indicated on Form SCD; Early Completion Bonus if Project Completion is achieved ahead of the Project Completion Date; Escrow Submission of Bid Documents for Successful Proposer. Liquidated Damages: BIN 5500870 increased to $7,000 per calendar day BIN 5500890 increased to $7,000 per calendar day Early Completion Bonuses Project Completion: $7,000 / day (50 days max)

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) ATCs Alternate Technical Concepts (ATCs) are proposals from the Proposer to design and construct the Project in a manner that is different than the Contract Requirements and which: are submitted to the Department for review, consideration and approval prior to Proposal submittal; have significant savings in time, and/or reduction of traffic impacts, and/or community disruption, and/or project costs, and/or environmental impacts, and/or other beneficial impacts to Project parameters; in the Department’s judgment offers an equal or superior Project.

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) ATCs (continued) Proposers may submit, on a confidential basis, any number of ATCs, using Form ATC, for the Department’s approval; The proposed end product of each alternative must, in the Department’s judgment, be equal to or better than the Work product described in the Contract Documents; Department may provide ‘Conditional Approval’ of an ATC; The Design-Builder is solely responsible for obtaining approvals from any local municipality, public agency or permit issuing agency for ATC concepts which would affect their facilities, operations or issued permit. If submitting more than 1 ATC, Proposer should indicate order of priority Proposers should advise the Department in it’s ATC if it believes a One-on-One is appropriate

Regions 8 Bundled Bridges (Ulster County) Design-Build Project (PIN 8BOW.0B; D900031) One-On-One Meetings One-On-One Meetings may be requested by proposers during the period stated in the RFP; If One-On-One meetings are held, at least one meeting will be offered to each Proposer; Department reserves the right to disclose to all Proposers any issues raised during the meetings that may materially affect the project; Department will not disclose information pertaining to an individual Proposer’s technical concepts, proposal or ATCs; Additional One-on-One Meetings may be held if required to review ATCs or if the Department believes it is warranted. One-on-One Meetings: Proposers to request meeting to be held during the established period.

Form NC – Non-Collusion Affidavit Form shall be signed and notarized There is no signature line; sign at bottom of the form. Form R – Summary of Individual’s Experience Information presented on Form R should match information provided on resumes (project title, time spent on project, etc.) Individual’s Total Time on Project should reflect total QUALIFYING time on project. Resumes should reflect individual’s actual duties on each past project, not just a general project description. Information should be consistent between resumes and Form R (project roles, dates, etc.) Place resumes and Forms in the order that they are requested in the RFP.
