The 2002 Assistance to Firefighters Grant Program’s on-line application process has been designed to lead fire departments through a series of screens that will ask basic questions about the department, its firefighting workload and capacity, and its proposed project. While the web-based process will be self-explanatory and easy to use, FEMA has developed this slide show to provide interested fire departments with an introduction to the application. Fire Departments will be able to complete an actual application at from March 1 – April 1, NOTE: The screens in this presentation are samples, and while the information requested will not change, the actual on-line application may differ slightly.

STATUS This screen allows the users to start an application, update saved applications, withdraw applications and read incoming mail from FEMA personnel. Applicants can also use this screen to grant or revoke access to their applications by other people on their staff or assist them with the preparation of their grant applications. To start or update an application select Update Application. Users can only complete one application. SAMPLE

APPLICATION STATUS This screen informs the users of the overall status of their application by allowing the users to see which portions of the application are complete, incomplete, or have not been started. SAMPLE

OVERVIEW This screen is used to obtain the name and address of the organization that is preparing the application. SAMPLE

CONTACT INFORMATION This screen is used to obtain the contact information of the authorized representative of the fire department and alternate contact information. SAMPLE

APPLICANT INFORMATION This screen asks the applicants for important basic information such as the name of the fire department, the type of fire department, the EIN (9 digit - tax identification) number of the fire department, etc. SAMPLE

APPLICANT INFORMATION (Continued) This screen also asks the applicants for their banking information, information about any other federal grants that they may have received and information regarding any delinquencies on federal debts. SAMPLE

FIRE DEPARTMENT CHARACTERISTICS (PART I) This screen will be used to gather important information about the fire departments. The applicants will be asked the type of department (career, volunteer, or combination), the square mileage of their jurisdiction, characterization of the response/jurisdiction area (urban, suburban, or rural), population of permanent resident population of their primary first response area or jurisdiction served, etc. SAMPLE

FIRE DEPARTMENT CHARACTERISTICS (PART I) (Continued) This screen will be used to gather important information about the fire departments. The applicants will also be asked the number of active firefighters, the number of stations in their department, if they are able to meet the cost-share requirement of the grant, if they currently report to the National Fire Incident Reporting System (NFIRS), and the types of services the department provides (EMS transportation and non- transportation, Fire, Hazmat, and Rescue). SAMPLE

FIRE DEPARTMENT CHARACTERISTICS (PART I) This screen will be used to gather more information about the fire departments. The applicants will be asked the number of fire related fatalities, the number of runs per year and information about the age and mileage of the firefighting vehicles the department currently has. SAMPLE

RESPONSE TO EMERGENCIES The applicants will be asked to enter the number of responses per year by category that the department has made (i.e. Structure fires, Vehicle fires, Vegetation fires, EMS and Rescue, Hazardous Condition/Materials Calls, Service Calls, Good Intent Calls, False Alarms, Other Calls and Incidents). SAMPLE

REQUEST INFORMATION The applicants will use this screen to choose the one program area for which they want to apply. The program areas are: Emergency Medical Services, Fire Operations and Firefighter Safety, Fire Prevention Programs and Vehicles. SAMPLE

REQUEST DETAILS This screen will be used to ask the applicants questions specific to the program area for which they apply. SAMPLE

TOTAL BUDGET This sheet is the total budget sheet that will be automatically completed after the applicant answers the questions on the Request Details page and adds the appropriate budget items. This sheet can be used by the applicants to review their overall budgets. On this page, the applicant will need to specify the source of the cash that will be used to meet the matching requirement (applicant, state, local or other sources such as fundraisers). SAMPLE

NARRATIVE STATEMENT The applicants will use this screen to enter their program narrative. The applicants need to explain in detail how the funds provided will be spent if awarded a grant. The applicants must also give detailed information that demonstrates the financial need for the assistance and how the costs expended under this program will benefit the fire department and/or public. This will be a major part of the evaluation process. SAMPLE

ASSURANCES AND CERTIFICATIONS The applicants must read and accept the Assurances and Certifications included on this page. SAMPLE

SUBMIT APPLICATION Once the application is complete, applicants will go to the Submit Application page to forward their applications to FEMA. Once an application is submitted it can not be edited. When an application is successfully submitted, the applicant will be able to view a Confirmation Page that will include the application number. After submission of the application, all applicants should print a complete copy for their records. SAMPLE
