School Nurse Information Hours: 8:30-2:30, Monday through Friday Trained in CPR, AED, and Epipen Provides minor injury care Ensures compliance with Immunizations Dispenses Medications Calls parents/sends students home when necessary Arranges talks on food allergies, nutrition, dental care, etc.

Medications Nurse can give both prescription and over-the- counter medications. Parents are asked to provide pain/headache medication when needed. Must have medication permit form for each medication. Medication must be in child’s name and in the original container Parents need to transport all medications

Immunizations Every student in public or private schools is required to comply with required immunizations before the first day of school. Indiana Code (IC ) Students who do not meet this requirement will be excluded from school.

Reasons your child may be sent home… Conjunctivitis (Pink Eye): Students may return to school with a written note from their physician stating that the eye redness is due to allergies or if they have a note from a physician stating they are on medications to treat the pink eye and they are no longer contagious. Eye should be clear. Fever: Students will be sent home if they have a temperature over 100 degrees. Child may return to school when they have been fever free for 24 hours and they are not on medicines to keep their fever down (Tylenol and Motrin type medicines). They can return to school on antibiotics. Vomiting / Diarrhea: Students will be sent home if they have vomited or have diarrhea at school. They should not come to school until 24 hours AFTER the last time they have had vomiting or diarrhea.

Uncontrolled coughing: Students should not come to school if they have a cough they can not contain or is not controlled. Students with an uncontrolled cough will be sent home. Unidentified rashes or parasites: If a student has an unidentified rash or issues with parasites, they may be sent home until the problem is identified by a physician and they are treated and /or cleared to return to school. These are not the only reasons a child will be sent home and it is at the nurse’s professional discretion if a child needs to stay at school or be sent home. A healthy child is ready and willing to learn. Staying healthy is something to be learned so education and health are intertwined. Your help in keeping your child home when needed will help us keep others healthy

Required Vaccines Five doses of DTaP: (diphtheria, tetanus and acellular pertussis). **Four doses are acceptable if the 4th dose was administered on or after the 4th birthday and at least six months after the 3rd dose. Two doses MMR: measles-containing vaccine (MMR combined measles, mumps and rubella) Four doses of polio vaccine: **Three doses of polio vaccine are acceptable if 3rd dose administered on or after child’s fourth birthday and the three doses are all IPV or all OPV. Three doses of hepatitis B vaccine Two doses of Varicella for kindergarten or statement that the child has had chickenpox already Two doses of Hepatitis A vaccine
