Common Deficiencies with Submitted Surveys Surveys are illegible Registration and equipment identifiers don’t match information on file with Dept. Diagrams missing key identifiers including operator position, exposure switch location, lead shielding if present; wall, floor,and ceiling construction. Missing information about the survey instrument used (i.e. calibration date, probe information, etc)

Bureau Policy for Deficient Surveys If survey deemed incomplete: – returned to the qualified individual who performed the survey for correction and re- submittal within two weeks If no response: –Letter sent to registrant explaining that the survey does not meet regulatory requirements and must be corrected within 30 days Enforcement Action if still deficient
