NOTE CARD FORMAT REQUIREMENTS: You will need Information Cards per subtopic if you have 5x8 size note cards. If you have 4x6 size note cards, you will need at least 15 Information Cards per subtopic. The final product of the note card assignment is to be able to hand in THREE piles of information cards (amount varies depending on card size, as explained above), using source cards to divide the information cards within each pile (each pile will represent a different subtopic). INFORMATION CARD PARAPHRASING & QUOTING Smith, Sean Causes of Lung Cancer Tobacco use, such as smoking cigarettes and chewing tobacco, is the leading cause of lung cancer amongst adults between the ages of 35 and 55. Over 25% of social smokers (those who smoke one pack a week) develop lung cancer before the age of 50. “Pregnant mothers who smoke increase the chances that their unborn children will develop lung disease or cancer by 80%.” (pg. 52) Exposure to second-hand smoke can endanger even non-smokers. Pgs Student Name NOTES: Remember that paraphrasing means to summarize a source’s (author’s) words AND ideas by putting them into your own words. Ask yourself, How would I explain this information to my friends or family? Failure to paraphrase effectively is considered plagiarism. When quoting someone from a source who is not the author of the source, remember to include that person’s credentials (what makes him/her an expert?).

The following slide contains a DETAILED LAYOUT OF NOTECARDS, including SOURCE and INFORMATION CARDS. The cards are arranged in order, so the SOURCE CARD on top of each of the three SUBTOPIC piles remains on top while the rest of the INFORMATION and SOURCE CARDS are placed underneath.
