As we work toward reducing readmissions to avoid hefty penalties issued by CMS, better care coordination is critical. Care coordination nurses are working with more complex patients to ensure they are following and understanding their treatment plans to avoid readmission. Here’s an example of a Medicare Value Partners patient who needs long term follow-up and a myriad of other resources. One of our care coordination nurses held weekly calls with the patient since April She has coordinated everything from home health and rehab for him to nutrition. He is very thankful and sends her pictures of himself and his family. In our story, the two meet face to face for the first time. Patient Story: Care Coordination in Action Play Video

Presence Health will have its own product on the Exchange. What is the public exchange? Under the Affordable Care Act, the public exchange was established for individuals and small groups to purchase insurance. Presence Health is working with Land of Lincoln Health Plan to offer this product. –Land of Lincoln is a health care cooperative formed as a result of Federal health reform legislation. Presence Health is developing a plan to encourage enrollment by targeting populations most likely to be seeking individual health insurance. Presence Health to have a product on the Illinois Public Exchange

Systemwide Flu Vaccination Deadline – Nov. 1 The influenza virus can affect employees, patients and visitors. Immunization has proven to be the most effective method to stop transmission of the virus. Employee Health Services (EHS) offers free flu vaccinations to all employees, physicians, medical residents and volunteers. A list of scheduled flu clinics and locations is available on the intranet. New medical exemptions need to be submitted to EHS for approval. Exemptions for religious reasons and reasons of conscience will again be available to employees seeking this type of exemption.
