Job Description Job Title: UPN(s): Division: Section: Location: Shift: Report To: Salary Table: Grade/Step: Salary Level: FLSA Status: SOC Code: W/C Code: Approved On: Revised On: Physical Activity Lifestyle Specialist SDP-05 – SDP-06; SDH-01 Community Services Division Diabetes Program Section Potawot Health Village, Arcata/Del Norte Monday – Friday, 8:00 am to 5:00 pm Diabetes Program Manager Table 1 – Line Staff Grade to $11.86/hr to $15.98/hr ($24,669/yr to $33,238/yr) Non-Exempt - Hourly April 4, 2005 SUMMARY: The primary purpose of this position is to serve as a lifestyle “coach” for participants in the intensive part of the UIHS Diabetes Prevention Demonstration Project (Competitive Grant Program of the SDPI). As such, the employee will develop relationships with the participants and use lessons and tools from the Diabetes Prevention Program (DPP) as well as other methods learned appropriate to our region and culture to facilitate lifestyle change for participants toward preventing diabetes and living a healthier, more active life. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. The Physical Activity & Lifestyle Specialist (PALS) will be responsible for developing a relationship with project participants to facilitate the intensive lifestyle intervention component of the Prevention of Diabetes Project grant for the participants. This will require a considerable interpersonal skill level. The PALS will serve as a facilitator between diabetes program staff and client. The PALS will maintain weekly contact at a minimum with their assigned participants in the active phase of the curriculum and then maintain regular contact with them at regular intervals (not less than monthly) to be specified at a later date throughout the course of the program. These contacts will encourage participation, answer questions, facilitate behavior change, and develop the relationship required for success in this program. The PALS will work closely with the project participants and family members to identify ways to increase exercise, and to utilize program “tools” to meet exercise goals. The PALS will help project participants achieve specific weight loss behavior change goals in a step-wise fashion using standardized strategies, tracking books and other DPP “tool box” strategies. The PALS will reinforce the [education curriculum] lifestyle balance core curriculum (developed out of the DPP curriculum) with participants and families as taught by the prevention program staff. The PALS will facilitate self-management using food records and physical activity habit logs. The PALS will assist in creating group activities to maintain motivation in the intensive lifestyle component of the project. The PALS will work with behavioral specialists and/ or receive special training to guide participants in behavioral change skills and behavioral reinforcement needed to make lifestyle changes to attain the nutrition and physical activity. The PALS will assist with the management, the “support materials” components of the project for participants. The PALS may need to travel extensively throughout the tribal health service area to meet with participants to achieve the goals stated, as well as to participate in, plan, and implement community-based activities and events. The PALS will document all relevant participant data for the study participants. Assists in the development and monitoring of all performance improvement initiatives and reporting requirements of the prevention program. Demonstrates positive attitude and professional manner, as well as appropriate problem solving and communications. Maintains cooperative and effective working relationship with diabetes program staff, other UIHS employees and supervisory staff, with tribal officials and visitors to UIHS. Meets all requirements of UIHS policy & procedure. Must have a willingness to learn on the job, employ cultural techniques, be trained in said techniques as indicated. The PALS will agree to participate in recruitment activities and lifestyle sessions that may occur within or outside regular working hours or on weekends. The PALS will be an integral part of a multi-disciplinary team (both the Diabetes Team and Prevention Team), and will work closely with the diabetes program staff to identify impediments to adherence and retention in the project. They may also be asked to participate in recruitment activities as needed. Must be willing to participate in the lifestyle balance program activities in order to serve as a mentor and example to help facilitate relationship and behavioral change for participants in the intensive program. Must be able to maintain strict confidence of all information received on the job. Participates in mandatory initial training and annual update training as indicated as well as continuing education as required. Other duties as assigned.
