Patient Self-Management in Rural Settings Collaborative effort in rural, northern Ontario community between Bell, UHN and Nortel Networks Chronic diseases are a growing challenge for our health and social care systems and affect 1 in 3 Canadians Bluetooth-enabled medical devices are used to transmit data to a mobile device which relays data back to a data repository where rules are applied sending alerts to a nurse practitioner or family physician as well as the patient by automated text or phone messages Demonstration of a technology that is applicable to a wide variety of chronic diseases and conditions such as heart failure, asthma and depression and builds on the demand for new and more effective ways to improve chronic disease management.
