You may or may not be able to complete all blocks on each form It’s acceptable to leave certain blocks blank if you are unsure of an answer or don’t have all the information you need to populate a field. Used to gather information on the deployment of existing GRP response strategies or potential new strategies (including Company Control Points). Used to record when strategy sites are visited. Additional notes can and should be attached to the form prior to submission. Photos (with a description and additional information) should also be included. GRP Field Work Report Form

If Off-Site, provide additional information (Where, Lat/Long, Size, site Contact Information) or complete a Staging Area & Boat Launch Development Form 11. Resources Requiring Protection: Provide information on the sensitive resources in the area that you know about (Natural, Cultural, or Economic); resources this strategy is meant to protect. If this is a GRP Strategy you can refer to the GRP for reference, but based on your knowledge, indicate anything that may have changed or anything that needs to be considered regarding resources in the area

GRP Field Work Report Form 12. Response Strategy Details: If this is a GRP Strategy and it was deployed as written in the plan, enter “as written.” If not, provide the type of strategy implemented and describe how it was deployed (major steps). Strategy Types: (One or More of the following) Diversion Collection Deflection Exclusion Notification Containment* *Source control and containment of a spill at the source are always a higher priorities than the deployment of GRP response strategies. Containment strategies are rarely identified in GRPs since it’s usually considered a separate action, something done before or concurrent with the deployment of GRP Response Strategies.

Types of Response Strategies

GRP Field Work Report Form 13. Equipment & Personnel: List the number and type of equipment and personnel used to implement the strategy. Equipment includes boom size and length (include segment lengths); type, number and size of anchoring systems used; number and size of work boats needed (if applicable); additional line/rope. Personnel includes the number of boat operators, supervisors, and trained laborers/workers. 14. Boat Required?: Check “Yes” if a boat was used to help deploy the strategy, otherwise check “No.”

Additional Comments: Include weather conditions (including wind speed & direction), wave height, tides, river speed, location on river or creek (river right or river left), and any other comments regarding the field visit or test that you think are important. There is only so much room on the form, so it’s acceptable to include other notes with the form on separate pages – referencing the appropriate block number related to your comments (Block #1 - #16).

Estimating River Speed Rule of Thumb: An object that drifts 100 feet in 1 minute is traveling at a speed of ~1 knot per hour 100ft x 60 minutes = 6000ft (1 nautical mile = 6080ft) If it travels 100ft in half that time (30 seconds) it’s moving twice as fast; ~2 knots.

Photographs Photographs: If available, provide photographs of the strategy as deployed. Include a short description of each photo, the date & time it was taken, the location (with lat/long), and direction facing (N, NW, W, SW, S, SE, E, or NE).

If you don’t know the distance, just enter “unknown.” If the site is not located on a river or creek, then leave the fields blank or enter “N/A.” 9. River Right or Left?: Enter the side of the river the site is located on. Looking downstream, River Right is on your right and River Left is on your left. Safety Issues: Record any safety concerns that should be considered when using this site; Slips-Trips-Falls, Near Roadway, Water Hazard, Wildlife Hazard (i.e. snakes, bees), vegetation (i.e. poison ivy), steep or unstable banks, falling rock, etc. 11. Site Access: Record if this site can be accessed by land, boat only, or either. Provide information on conditions that might limit access to the site; Locked Gate, Overgrown Vegetation, Private Property, Poor Road Condition, 4x4 Access Only, etc. 12. Property Contacts: Provide name and contact information for person, group, company, or agency that can provide access to the site. GRP Strategy Development Form

13. Staging Notes: Record if equipment needed to deploy a strategy at this location could be staged “on-site” or “off-site.” Is there enough room (and access) to stage equipment on-site or would an off-site location be needed? If off-site, where would you stage equipment at? If possible, complete a Staging Area & Boat Launch Development Form for any off-site staging area proposed. 14. On-Scene Conditions: Record weather conditions (including wind speed & direction), wave height, tides, and river speed (if on a river or creek). [100ft/1min = 1kt] GRP Strategy Development Form

15. Waterbody Info: Record the waterbody name and the shoreline composition (especially areas at/near potential shoreside anchor points). GRP Strategy Development Form 16. Proposed Strategy: Enter the type of strategy you propose for this location. If based on your judgment and experience you believe the site has no potential as a strategy location, enter “none” or “no potential.” If you are unsure, just leave the field blank or enter “not sure.” 17. Strategy Objective: Enter the purpose for implementing a response strategy at this location. What would be the benefit of having a strategy here? What resources would be protected?

18. Implementation: If you believe a response strategy at this site is “doable” then describe the major steps needed to implement a strategy at this location. How would you (or could you) implement a strategy here? If you are unsure, just enter “not sure” or leave the field blank. GRP Strategy Development Form

19. People & Equipment Needed: Enter the type and number of people it would likely take to implement this strategy. How many supervisors, trained laborers, and/or work boat operators? Enter the type and amount of equipment that you think it might take to implement the strategy you propose. How much boom (in feet)? What size or type of boom? How many anchoring systems? The type and weight of anchors that might be used (i.e. 43lb Danforth)? How many shoreside anchoring posts? How much line/rope? GRP Strategy Development Form

20. Anchor Points: Enter the location or latitude/longitude (in decimal degrees if possible) for the shoreside anchor point or points where you would/could secure the boom to shore. 21. Photographs: Record the date and time the photograph was taken, location and/or latitude/longitude (in decimal degrees if possible), and the bearing or direction the photographer was facing when taking the picture (N, NW, W, SW, S, SE, E, NE). Provide a brief description of the photo (one or two sentences is appropriate). GRP Strategy Development Form

22. Site Notes and Sketch: There is only so much room on the form, so it’s acceptable to include other notes with the form on separate pages – referencing the appropriate block number related to your comments (Block #1 - #23). A sketch of the site and any proposed response strategy should be included with notes. The sketch should include landmarks and a reference for distance. It should clearly show the booming strategy you propose, shoreline boundaries, anchor points, “on-site” staging areas, boat ramps, and nearby facilities (i.e. restrooms, parking, covered spaces, fenced and gated areas, etc.) GRP Strategy Development Form

Site Sketch Either a hand-drawn sketch or a diagram using internet tools (i.e. google, bing, open street maps, etc.) is acceptable. Ensure you don’t violate copyright restrictions if you decide to use internet tools to develop a diagram.

23. Check All Applicable Boxes: Put a check in all appropriate boxes for Proposed Strategy Type, Resources Potentially at Risk, and Waterbody Type. For Strategy Type “Other” you can enter Culvert Block, Underflow Dam, Containment, Sorbents/Sorbent Boom or Snare (Pom-Poms), as appropriate. Again, it’s acceptable to leave certain blocks blank if you are unsure of an answer or don’t have all the information you need to populate a field. GRP Strategy Development Form

Additional notes can and should be attached to the form prior to submission, especially since there won’t likely be enough room on the form to record everything you need to document. Boat Ramp Available? Enter “Yes” or “No” in the first column block. In the second column block enter how many ramps/lanes are available at this location. Boat Ramp Type: Enter the type of payment material for the boat ramp in first column block (concrete, asphalt, gravel, dirt, etc.). In the second column enter the grade of the boat ramp - steep (15˚+), moderate (12˚-13˚), gentle (<7˚). Boat Dock? Is a boat dock available at or near the ramp? Enter “Yes” or “No” in the first column block. Enter the number of docks available in the second column. Boat Refueling Available? Enter “Yes” or “No” in the first column block. In the second column enter hours fueling available and kind of fuels (gas & diesel or gas only?) Block 10

Restrooms Available? Enter “Yes” or “No” in the first column block. In the second column, enter how many bathrooms or toilets are available and, if possible, the type (water flush, honey bucket, fixed pit, etc). Power Available? Enter “Yes” or “No” in the first column block. In the second column block enter the type of service available for use (15amp, 80amp, 200amp, etc). Water Available? If drinking water (potable water) is available at the site enter “Yes” in the first column block - if not available, enter “No.” Enter additional information in the second column block or N/A. Block 10

Parking for Cars Available? Enter “Yes” or “No” in the first column block. In the second column block enter how many parking spaces for cars and trucks are available and indicate if the spaces are marked (striped). Parking for Trailers Available? Enter “Yes” or “No” in the first column block. In the second column block enter how many parking spaces for trailers are available and indicate if the spaces are marked (striped). Waste Disposal Available? Enter “Yes” or “No” in the first column block. In the second column enter the type(s) of waste disposal available: garbage, sewage pump- out, oil/bilge waste pump-out, or other. If waste disposal is not available at this location, enter “N/A” in this block. Block 10

Telephones Available? Enter “Yes” or “No” in the first column. In the second column block provide any additional information of value or enter N/A. Cell Phone Coverage: If you have coverage at the site on your cell phone, provide the name of your cell phone carrier (T-Mobile, Verizon, AT&T, etc) in the first column block. In the second column block enter the strength of the signal (number of bars). Enter data speeds available if possible (3G, 4G, etc.). If there is no cell phone coverage at the site, enter “none” or N/A. If not check, leave this field blank. Block 10

Estimated Lot Size: Provide information related to lot dimensions in this first column. Provide the estimated lot area in the second column block. Primary Lot Cover: Determine the type of lot cover for the site (what the ground is primarily covered by) and enter it in the first column block (i.e. concrete, dirt, asphalt/blacktop, gravel/rock, dirt, grass, clay,sand, or “other” material). In the second column enter the percentage of total site area covered by the primary lot cover (i.e. 40%, 80%, 65% - your best estimate). Block 10

Covered Spaces Available? Enter “Yes” or “No” in the first column block. Enter the number of buildings or covered spaces available at this location, and the estimated total covered area available (SqFt) in the second column. Perimeter Fence/Gated? Enter “Yes” or “No” in the first column block. Provide additional information in the second column block (i.e. gate locked, gate open during daylight hours only, area fenced but not gated, etc.). User Fee Required? Enter “Yes” or “No” in the first column block. Enter the fee amount (in dollars $$$) and who (what group, municipality, organization, or agency) is collecting the fee (if known). Block 10

11. Access Issues/Concerns: Enter information related to access limitations for the site (i.e. gated, steep hill, dirt road, access hours, overgrown vegetation, muddy, 4x4 vehicle only, poor road conditions, etc.). You can also use this block to record any additional concerns you have related to site access. 12. Property Contacts: Enter the name and contact information for the property owner or the person that should be contacted in order to gain access to the site. If possible, record the person’s name, company name, address, telephone number, and – whatever the person or company is willing to disclose. Also verify contact information for late night/early morning access, and visits to the site on weekends and holidays. GRP Staging Area & Boat Launch Development Form

13. Photographs: Record the date and time the photograph was taken, the latitude/longitude (in decimal degrees if possible), and the bearing or direction the photographer was facing when taking the picture (N, NW, W, SW, S, SE, E, NE).
