 PQRS is a reporting system that uses a combination of incentive payments and payment adjustments to promote reporting of quality measures by Eligible Physicians (EP)  The program provides an incentive payment to practices with EPs who satisfactorily report data on quality measures for covered Physician Fee Schedule (PFS) services furnished to Medicare Part B Fee-for-Service beneficiaries  Allows EPs to assess their quality of care Source: CMS.gov

 EPs and Group Practices may choose to report quality information through one of the following methods: Individual EPsGroup Practices Medicare Part B ClaimsQualified PQRS Registry Web Interface (for groups of 25+ only) Direct EHR using CEHRT CEHRT via Data Submission Vendor Quality Clinical Data Registry (QCDR) CG CAHPS CMS-certified survey vendor (for groups of 25+ only) Source: CMS.gov

 The types of measures reported under PQRS change from year to year and generally vary by specialty  When selecting measures for reporting, EPs should consider factors such as: ◦ Clinical conditions commonly treated ◦ Types of care delivered frequently ◦ Settings where care is often delivered ◦ Quality improvement goals ◦ Other quality reporting programs in use or being considered Source: CMS.gov

 Individual EPs who meet the criteria for satisfactory submission of PQRS data for services furnished during the 2014 reporting period will qualify to earn an incentive payment equal to 0.5% of their total estimated Medicare Part B PFS allowed charges for covered services furnished during the same reporting period Source: CMS.gov

 EPs who do not satisfactorily report data on quality measures for covered professional services during the 2014 PQRS program year will be subject to a 2% payment adjustment to their Medicare PFS amount for services provided in 2016 Source: CMS.gov

 In 2014, EPs have the opportunity to earn the PQRS incentive and an additional incentive of 0.5% by working with a Maintenance of Certification Entity  Required: ◦ Satisfactorily submitting data for a 12-month period ◦ Participate in a Maintenance of Certification Program and successfully complete a qualified MCP practice assessment Source: CMS.gov

 Nassau University Medical Center (NUMC) – GNYHA ◦ Approximately 200,000 charts or 20 million images scanned in 4 months, both on and off-site ◦ 4-year contract for inpatient, outpatient, and ER records. Still in progress.  St. Christopher’s Hospital of Philadelphia ◦ Sub-contractor for Ricoh ◦ Approximately 110,000 outpatient charts, or 10 million images to be scanned and abstracted ◦ 15 out of 29 practices completed

 Lourdes Medical Associates ◦ Approximately 45,000 outpatient charts or 6 million images scanned and abstracted from 16 practices  Aria Health Physician Services ◦ Approximately 100,000 outpatient charts to be scanned and abstracted ◦ Still in progress: 40+ practices to be completed

 Langhorne Physician Services ◦ Approximately 56,000 outpatient charts or 7.5 million images scanned and abstracted from 5 practices ◦ Migrated approximately 16,000 charts from AllScripts EMR system into NextGen

 Memorial Sloan-Kettering ◦ 12,000 medical employee files, 1.5 million images ◦ Each folder indexed by employee ID number, employee name, scan date, box number, and document type ◦ Final images are provided in MedGate importable format ◦ Scanning was completed on-site, on time, and under budget  HR Project ◦ Scanned and indexed approximately 1.3 million HR images ◦ Over 71 million characters keyed from 29 different document types ◦ Input media sources were microfilm and original paper documents ◦ Project still in progress
