Raytown Schools Wellness Program Draft 7/10/10 Raytown Schools Wellness Program BCBSKC – A Healthier You Program Components: Assessment: Onsite health screening events BMI Blood Pressure Total Cholesterol/HDL Glucose Waist Circumference Body Fat Percentage Health Risk Appraisal Tools: On Site & webinar education classes ePHIT online modules Telephonic Lifestyle Coaching Online education resources Prevention programs Health Companion – Disease Management Case Management Onsite Wellness Coordinator Face-to-Face Lifestyle Coaching A Slimmer You Weight Management Program Rewards: Points to Blue Incentive Program Reporting: Aggregate results from HRA & Health Screening Annual Reporting Incentive Requirements Considerations: 2010-2011: effective July 1, 2011 A $25 charge per month if they do not participate in the following: Employee- Complete online Health Risk Assessment (HRA) Complete the biometric screening and/or receive an annual physical Participate in one Online Coaching activity Spouse- Complete online HRA 2011-2012: effective July 1, 2012 A $35 charge per month if they do not participate in the following: Receive an annual physical Be tobacco free or complete a tobacco cessation program 2012-2013: effective July 1, 2013 A $35 charge per month and a $200 prescription deductible will apply if they do not participate in the following: Participate in one wellness activity 2013-2014: effective July 1, 2014 A $35 charge per month and a $300 prescription if they do not participate in the following: Same requirements as 2012-2013 Wellness Activities 2010-2011: January 2011 –On Site health Screenings and HRA BCBSKC Online Coaching activities Webinars Onsite Classes Start communicating Tobacco Cessation opportunities and incentive requirements 2011-2012: Online HRA and annual physical campaign Online and Onsite Tobacco Cessation opportunities 2012-2013: Continue with 2011-2012 activities 2013-2014: Continue with 2013 activities and make modifications as necessary BCBSKC Onsite Wellness Coordinator Responsibilities Work to identify baseline (starting point) health risks of the population and utilization of preventive care services Understand the complexity of the district (multiple buildings, etc.) when addressing health risks Targeted webinars Provide turn-key solutions Facilitate health screenings Attend new teacher training District Wide Competition: For January 2011: The department that has the highest percentage of completion rate for the Health Risk Assessment & On Site Screenings will be eligible for cash prize for their department 1st place = $2,000, 2nd place = $1,000, then four $500 prizes (funds provided by the BCBSKC Wellness Fund at no cost to the district) Points to Blue Incentive Components: Members can earn points for: Taking Health Risk Assessment, screenings, classes, calling health coach, using ePHIT tools Points: Earn up to 250,000 points per year equivalent of $250 Redeem points for gift cards Other Activities: Classroom program for marketing the wellness program to the district employees Wellness blogging Raytown Education Foundation Walk - $3 per person contribution from the Wellness Fund to the foundation.
