Formed in 2011, we are the largest Catholic health provider in Illinois –12 hospitals –29 long term care and senior residential facilities –50 + primary and specialty care clinics –6 home health agencies –hospice, private duty, comprehensive Behavioral Health services –and more! 2

Our new name also has meaning related to the communities served by our healthcare system who rely on our presence to ensure quality, accessible health care for all.
