The mission of God’s Love We Deliver is to improve the health and well-being of men, women and children living with serious illnesses by alleviating hunger and malnutrition. We are dedicated to cooking and delivering the specific, nutritious meals a member’s severe illness and treatment so urgently require. Community Partners Program Our Mission

For 30 years, God’s Love We Deliver has been the NYC metropolitan area’s only provider of nutritious, home-delivered meals for people who are too sick to shop or cook for themselves. We have never wavered from our core principles. As being sick and hungry is a crisis that demands an urgent response: We deliver each client’s first meal on the next delivery day We never have a waiting list We never charge clients for their meals Core Principles

God’s Love is the only organization that:  Focuses on people with serious illnesses  Cooks and home-delivers individually- tailored nutritious meals  Provides services free of charge without regard to income  Programs include: Home Delivered Meals, Nutrition Services, HIV Grocery Bag Program, Community Partners/Managed Long Term Care God’s Love Is Unique

6 Registered Dietitians (RDs) on staff Medical nutrition therapy and nutrition education and counseling to Clients Families Caregivers Unique Nutrition Plan Our RDs work with clients to develop a unique nutrition plans to help manage their illnesses, medications and side effects, and to promote wellness Tailored Meal Program Menus are designed by our Dir. of Culinary Services, our Director of Nutrition Services and our RDs Nutrition: Our Signature Difference

Nutritional Care for Metastatic Breast Cancer Patients Our Nutritional Care for Metastatic Breast Cancer Patients program supports metastatic breast cancer (MBC) patients with enhanced services, including up to 21 home- delivered meals weekly, nutrition counseling, and referrals to supportive services.

 Meals can include: Breakfast/Lunch/dinner combination or Lunch only (including food for weekends)  Deliveries can happen: Twice weekly or once a week (Ex. Twenty one meals for 1 week delivered on Thursdays.) Meals / Meal Delivery

Our Clients We currently serve more than 300 women and men living with breast cancer -- approximately 25% of whom are living with metastatic breast cancer -- along with their caregivers and their children. Our breast cancer clients are: 97% female 42% over 65, 39% 50-64, 18% % Black, 29% White, 19% Latino, 3% Asian 80% with Medicaid/Medicare, 9% with managed care plan, 11% with private insurance, ADAP Plus, no/unknown insurance

Identify Indicators that a member requires immediate nutritional intervention include: Recent hospital discharge without supports at home Weight loss Cognitive impairment or serious mental illness Medically at risk General fatigue Difficulty chewing and/or swallowing Difficulty walking, getting around, shopping or cooking limitations

Refer We will work with you to provide: Flexibility with delivery days and requested service adjustments Customized menu based on diagnosis, medication interaction and dietary needs Streamlined authorization process Responsive customer service care for your plan and members

Nourish All of our meals are low sodium and are freshly cooked with no preservatives, starters or fillers. We provide meals by addressing a combination of restrictions, resulting in almost infinite meal variety for members. Meal restrictions include: Pork, Beef, Fish, Vegetarian Sugar, Fat, Dairy Renal, Minced, Pureed Acid/Bland and Fiber/Gas

Client Services Department  The team is a staff of 9  Provides support services to clients, caregivers, providers of medical and social service agencies as well as the staff and members of Managed Long Term Care agencies.  new clients enrolled each month  All clients must provide us with some type of Emergency Contact to reach in the event of an emergency  Maintains partnerships with over 180 of medical and social service agencies across the city
