CHW Definition A Community Health Worker (CHW) is a frontline public health worker who is a trusted member of and/or has an unusually close understanding of the community served. This trusting relationship enables the CHW to serve as a liaison/link/intermediary between health/social services and the community to facilitate access to services and improve the quality and cultural competence of service delivery. A CHW also builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. Barriers to CHW Sustainability Lack of Statewide or National CHW Standards – Includes core competencies, training requirements, curriculums, payment structures, etc. Unstable Funding – No Medicaid reimbursement, grants, insufficient job security and training due to time-limited grants Lack of Recognition for What CHWs Do – By providers, by funders, by the state, by the public

Solutions to CHW Sustainability Standardization of the CHW Profession – Certification, higher education-based training, career ladders, streamlined curriculum and competencies Policy Change & Funding Stabilization – Includes Medicaid reimbursement, Medicare, direct reimbursement, provider and system buy-in Legitimization & Support of the CHW Profession – Statewide recognition, raising awareness, mapping existing CHWs and programs, increased training programs for continuing education
