Common Challenges- Hospital and Community Health Needs Assessment Charitable Hospitals must complete Community Needs Assessment At least once every three years; first one must be completed by end of tax year beginning after March 23, 2012 Include input from persons who represent the broad interest of the community Include input from persons having public health knowledge or expertise Make assessment widely available to the public Adopt a written implementation strategy to address identified community needs Failure to comply results in excise tax penalty of $50,000 per year

Secondary Data: Core set of indicators has been developed with input from all partners KDHE will maintain health data to HCI for display through the web portal, data will be reviewed and updated quarterly. Under this proposal, unlimited number of indicators that will be maintained by KDHE. HCI will provide indicator templates (contextual information/metadata) that will assist users in working with the data. HCI will maintain 30 core non-health indicators (economy, built environment, education, transportation) for the state The system will support the upload of indicator data at the state, county, and public health preparedness region levels. Data that is uploaded can be viewed on a dashboard by the specified geographic unit.

Primary Data Local Resources- The system has the functionality to integrate a local resources and services directory data file on State central page and display as related content on the site. This feature is expected to be added sometime in 2012 and will be made available at no additional charge once generally available a part of the HCN system. KSU, KHA, and KALHD are working on a related project to provide assistance in developing primary data with Kansas 211 Community Poll Feature: feature allows local administration to post a poll question and get user responses.

HCI will provide meta data context for describing the indicator and why it is important KDHE in providing data, will initially provide data sets for several years so that trends can be communicated in the website The indicator page will also provide a link to the KIC website where additional drill down into the data can be accomplished The system will also provide comparison with state average through the dashboard approach. The system will also provide a Health People 2020 tracker- automated report dynamically shows where key national health targets have and have not been met. Prioritize Issues System provides for Custom Web Pages- allows local administrators to create basic web pages without any knowledge of HTML including "about us," “contact us,” topical reports, privacy and editorial policies With this functionality, the web portal will include tools and links for resources for conducting community health assessments. The project management team/partnership of KHI, KDHE, KALHD, and KHA will be responsible for identifying/developing the content of these resource pages. Some of these resources will include tools to assist teams in prioritization of issues Community ToolboxCatholic Hospital Association’s ToolOthers as Identified Links to other resources can include:

Team Communications Collaboration Forums – this feature allows local administrator to create pages with specific content from any part of the site and add new content in order to showcase an issue or theme. Module supports online forums (and list serve) to enable collaboration on a project or issue area. Forums may be closed (limited to a specific list of participants) or open to the public. Report Assistant – enables rapid development of topical local reports

Public Communications The system is designed to provide a web based portal for community health assessment information. The link to the county list of indicators can be embedded within existing or new local websites, without restrictions. Not included in this proposal, is an independent web site for each community. Featured Content – ability to rotate local content/stories based on client preferences Administrative Tools – clients have password protected access to a wide range of administrative tools that allow for unlimited local content to be added, including indicators, promising practices, featured stories, reports, and initiative pages to highlight local programs.

Evidence Based Interventions Promising Practices – promising practices provided by HCI. Scope covers 8 taxonomy areas; each practice is classified as good idea, effective practice or evidence-based practice. The system links promising practices to particular health indicators displayed through the system. So the system provides a list of promising practices related to a specific indicator or subject area. Administrative Tools – clients have password protected access to a wide range of administrative tools that allow for unlimited local content to be added, including indicators, promising practices, featured stories, reports, and initiative pages to highlight local programs.
