FINAL FINANCIAL AID AWARD Reprint this from the school website or make a copy of the letter you turned in. If you had changes made to your award package, make sure you submit the most up-to-date letter.

CONFIRMATION OF COMPLETED ENTRANCE LOAN COUNSELING AND MASTER PROMISSORY NOTE Once you complete both Entrance Counseling and your Master Promissory Note, you will print the confirmation page. Directions for completing Entrance Counseling and the MPN are on Google Drive.

COPY NATIONAL CLEARINGHOUSE WAIVER Counselors will pass this out for you to sign and put in your folder.

COPY OF IMMUNIZATION RECORDS You must come back after graduation to pick this up with your final transcript. YOU WILL NOT BE A COLLEGE STUDENT until your school has these records!

COPY OF HEALTH INSURANCE CARD You will need to get this from you parents, bring it in, and copy it here at school. If you do not have a health insurance card, your school will require you to purchase health insurance from them.

FRIEND MUCHIN GRADUATES Search Muchin Graduates on Facebook and friend the page. The MCP Alumni Coordinator will keep you posted on important college matters and update you on scholarships and FAFSA. You can contact the Alumni Coordinator through Facebook if you need support next year.
