M AJOR E MERGENCIES If it is not practical to move the ill or injured individual, do NOT call 911; call the Rice University Police Depending on the injury, individual will be taken to Hermann Hospital’s Emergency Room (or hospital of choice); Rice ID required If injury involves a chemical, an MSDS should accompany the victim to the hospital

Report immediately to the supervisor, who should then fill out a First Report of Injury form M INOR E MERGENCIES If medical attention is required, the injured should be taken to Hermann Hospital’s Emergency Room or their physician of choice

For situations not involving an injury contact the Environmental Health and Safety Department (EH&S) Complete an Accident Incident Report Form Submit it to the Department Head and EH&S R EPORTING A CCIDENTS
