Engagement & Recovery Team An overview… The team consists of 3 Outreach workers and was launched in November 2010 Objective: to increase throughput and positive exits –Re-engagement of potential ‘unplanned exits’ –Targeted ‘Recovery Reviews’ –Residential Rehab preparation group –Aftercare provision The team work concurrently with the existing care coordinators
Engagement & Recovery Team All 3 have excellent IT skills which have allowed the increased use and interrogation of client data Each cover a specific locality and have worked there for a number of years Attend weekly team meetings in local CDT’s Weekly Peer Supervision, monthly PSI supervision Senior Recovery Worker Drugs Worker / Expert by Experience Recovery Worker Drugs Worker Recovery Worker Training & Employment background
Re-engagement (that sticks...) Aims… to re-engage back into meaningful treatment exit drug free / occasional user Through… Telephone, , text message, home visits etc… Therefore… Reducing unplanned exits & increasing service user ownership of their treatment journey Of the 36 clients re-engaged in the last 6months, only 1 has dropped out of treatment since
