The efficiency and usefulness of an organizational paperwork system and the communication of those procedures to its staff is critical to the operation of the facility. Recently, a large hospital system reached out to Proforma for assistance with redesigning their paperwork and developing a process for emergency room patient care and discharge. The patient information forms the hospital had been using for emergency room admission, care and discharge were no longer useful. They did not contain the necessary information the doctors and departmental staff needed to deliver quality care. They were difficult to understand and required too much time to fill out accurately. The hospital staff needed a form that would be effective, efficient and accurate to streamline the admission and discharge process. Some people’s challenges create other people’s ideas. Proforma went to work.

SUCCESS WITH BUSINESS DOCUMENTS SOLUTIONS SOLUTIONS To address all of the necessary components required by the staff, it was determined that the best way to redesign their paperwork was to convert the existing form into two separate documents. The Admission Form would be reduced from a 3- part 22” form to a 2-part 11” form, saving money and time. The Patient Care Order sheet, the real benefit to the new system, now allowed the staff to provide patients with a discharge receipt and medical orders upon discharge. On the back of the form, a 3-part, 7-5/8” snap set transferred the discharge instructions to another part of the form using a special print coating. Once completed, the hospital would be left with a neat, 8-1/2 x 11” sheet, three-hole punched and ready for filing. RESULTS RESULTS The end result was an easy-to-use, understandable form that served the needs of the emergency room staff. It allowed them to provide the patients with a precise account of their patient care, follow-up procedures and home-care medical instructions.
