2 Qualifications for Paid Leave Employee must be in a qualified benefits eligible position Research grant employees must be eligible for earned leave in accordance with the grant agreement
3 Types of Qualified Paid Leave Annual Sick Holiday Other Leave with pay
4 Annual Leave Accrual years: 15 days a calendar year = 9.38 hours a month 11 years plus: an additional 1 ¼ day increase for each year Usage May use up to 30 days of annual leave in a calendar year Department approval required
5 Annual Leave Carry-over Maximum 45 days will be carried forward to the next year (at the end of the calendar year) Donation to Leave Pool Maximum of ½ of your accrued leave each year May not be designated to a specific individual
6 Sick leave Accrual 15 days per calendar year = 9.38 hours a month Usage May use up to 10 days of sick leave to care for an immediate family member Sick Leave in excess of 10 days requires HR approval Leave taken pursuant to this policy may qualify as FMLA leave and, if so, will run concurrently.
7 Sick Leave Carry-over Maximum 180 days will be carried forward to the next year (at the end of the calendar year) Donation to Leave Pool Maximum of ½ of your accrued leave each year May not be designated to a specific individual
8 Paid Holiday 12 paid holidays Observance of paid holiday may differ within School of Medicine departments (Check with HR for observance schedule)

11 Military Leave with Pay Long Term Military Leave (Up to 5 years) Must be part of Operation Enduring Freedom or Operation Noble Eagle May use 45 days of annual leave each year May use 90 days of sick leave each year Must be returned to active payroll status each year
12 Disaster Services For American Red Cross Volunteers May use 10 days as paid leave per calendar year Requires HR approval
13 Blood Donations Employees are permitted to donate blood during normal working hours without being charged leave.
14 Bone Marrow Donation Employees may use up to 40 hours of leave with pay to donate Bone Marrow May require physician/medical documentation and certification Requires HR approval
15 Organ Donation Employees may use up to 30 days of leave with pay May require physician/medical documentation and certification Requires HR approval
16 Court (Jury) Leave Employee summoned to court or jury duty will be granted leave with pay Important: See policy for additional requirements and qualifications
17 Bereavement Leave Employee is granted up to 3 days with pay for death of an immediate family member Important: See policy for definition of immediate family member and additional requirements.

19 Family Medical Leave Act (FMLA) Eligibility Must have been employed with the University for a minimum of 12 consecutive months Worked at least 1,250 hours Qualification Serious health condition Self Immediate Family Member Your Spouse, Your Dependent Child, or Your Parents Birth or adoption of child
20 Family Medical Leave Act (FMLA) Entitlement 12 weeks per calendar year 26 weeks for injuries and/or illness related to military service
