Community Health Needs Assessment by: 1)Identifying significant health needs of the community 2)Including requisites for improvement or maintenance of health status in the community at large, neighborhoods or populations experiencing disparities. For example, the need to address financial and other barriers to accessing care, to prevent illness, to ensure adequate nutrition or to address social, behavioral and environmental factors that influence health. 3)Hospitals must identify measures and resources available to address such health needs.
