What is Goal Attainment Scaling? Scoring system to assess/report on outcomes Clients make the goals in collaboration with providers Scores based on goal outcomes 5 levels of outcomes
Five Point Scale
Five Point Scale Increase mobility and weight bearing capacity Walk around the block two times per week with use of an aid at 3 months Walk around the block once per week with use of aid at 3 months Walk around the block three times per week using the aid sparingly at 3 months Cannot walk around the block at all with or without an aid at 3 months Walk around the block three times per week without an aid at 3 months
Rehabilitation Assessment Life Satisfaction Indicators (LSI) are identified by client LSI creates a snapshot of the client’s satisfaction with life LSI results assist with the Rehabilitation Assessment
Rehabilitation Plan with Scaling Goals are made for Rehabilitation Plan based on Assessment Up to 4 Goals 5 levels of outcomes are defined for each goal and activities identified
Plan Progress Goals are reassessed as required LSI is completed 6 monthly
Plan Closure Final score given for each goal A final LSI is completed Success = expected outcome (0) or better (+1,+2)
Expected Benefits Better engagement with clients Clearer plan for clients Clients take ownership of their goals Clients have better understanding of rehabilitation process
Effects on Current Process Minimal impact on DVA Staff, as providers establish the goals with clients and monitor progress Once familiarity with process is established, minimal cost impact
