RE-CERTIFICATION PROCESS Eric Cheong 2015

Re-Certification Process 2014 Statistics: 768 certified members. 284 re-certification each year. 142 certified members each 6 months 142 x 90% x 15 minutes = 32 hrs 142 x 10% x 45 minutes = 10.7 hrs 90% of all re-certification peak at June and December How you can help me to process your re-certification quickly ?

1) Logistics: Regular mail couple months ahead of expired date. Courier and registered mail require more time and delay. Correct address and not to Lakeshore Blvd. 2) Application Journal: Application form incorrect version – have to be August 2013 or later Incorrect certification number and/or expired date. Written information is illegible and/or inaccurate. No check, no money order nor credit card payment. Credit card payment without expiry date or signature. Journal application form not signed and dated.

Re-Certification Process Document / Records A letter of employment is needed while a business card or pay record is not sufficient. Certificates, report cards, diplomas, etc are needed to claim your student credits. A letter ( ) from your committee chair describing your duties and services is needed for your committee work credit. All courses for instructor credit (s) must be taught IN ADDITION to your job responsibilities (if you are a full time trainer at your work, you cannot claim training credits within your company) Self-paced studying/preparation does not qualify for RU credits in student course credit.
