Central District Priorities to Update the District Public Health Improvement Plan Central District Coordinating Council January 22, 2013

Priority Setting Steps: 1.Review Background Data 2.Review Purpose & Goals 3.Establish Criteria for ‘Judging’ Potential Options

Overall goals for the DPHIPs: Strengthen the local/district public health infrastructure and improve performance on priority EPHS model standards Increase effectiveness & efficiency of public health strategies that will impact the Population Health Indicators Address other district priorities if identified

What does it mean to be a DPHIP Priority? 2011Guiding Principles: Maximize impact & use of limited resources Use evidence based strategies & population-based interventions Involve multiple sectors Address district disparities Strengthen & assure accountability

What does it mean to be a DPHIP Priority? Should we use others like collective impact pre-conditions criteria?  Influential champions = core leadership for workgroups  Sense of urgency = data, community/partner concern & energy  Adequate resources = workgroup participation & backbone support

Priorities selected for the Central District Public Health Improvement Plan (DPHIP) Population Health Priorities: Based on State Health Plan/District Performance Report all ( = connected to infrastructure) Public Health System/ Infrastructure Priorities: Based on Local Public Health System Assessment EPHS 3: Inform, Educate, and Empower People about Health Issues EPHS 4: Mobilize Community Partnerships to Identify and Solve Health Problems EPHS 7: Link People to Needed Personal Health Services and Assure the Provision of Health Care when Otherwise Unavailable EPHS = Essential Public Health Services

Central DCC 2011 DPHIP Goals: 1.Link people to primary care in the district 2.Link people to behavioral health services in the district 3.Build district capacity for integrated behavioral health (includes substance abuse & mental health) and primary care 4.Better educate and engage users of health care in the district 5.Strengthen partnerships with groups and service providers that work with youth in the district 6.Develop district system to coordinate and distribute important public health information 7.Develop a communication and mobilization network through the DCC to support vaccination efforts and improve vaccination rates in the district
