World Café Group Session

Introduction to world café sessions At the last session we looked broadly at what integration needs to achieve across a range of areas. To respond to your feedback, in this session we want to focus much more on the person at the centre We want to look at what we need to deliver and how we achieve that for a range of people with varying types and levels of care and support needs. At each of your tables is a case study and a diagram with 10 key aspects of delivering services. For your case study we want you to consider –what jointly we are doing well now –how we might improve on these areas through integration
