Processes That Lead to Interventions Request for Intervention Assistance Who can request? Parents, Teachers, Other Personnel, and People in the Community Student Support Team Development of Interventions for the Classroom (Intervention Plan) Who might be on this team? Parents, Teachers, Other School Staff, Outside Agency Staff, Advocates, etc. Additional Meetings to Monitor Effectiveness of Interventions & Determine Need for Further Actions

Evaluation Design Student Support Team (SST) determines Need for Formal Evaluation SST Sends Invitation to Parent SST Designs Evaluation SST Obtains Consent for Evaluation Evaluation Completed & Reviewed in Team Meeting

Surrogate Parent Basic Training: Nuts & Bolts of the Surrogate Assignment Process

Surrogate Parent Assignment Process A Surrogate Parent is required when no parent can be identified, when the whereabouts of a parent cannot be determined or when a student is a ward of the state. Determination of Need for a Surrogate  At Student Enrollment Principal Requires Parent Information  If No Parent Identified then Principal Requests Assignment of a Surrogate OR  Student in the Custody of Department of Social Services (DSS) so Parent Information Reviewed Parental Rights Terminated or Planned to Be Terminated Foster Parent, Relative or Custodial Relation Meets Definition of Parent  If No Parent then DSS Requests Assignment of a Surrogate Parent

Surrogate Parent Assignment Process Cont. Surrogate Parent Training Components:  Surrogate Parent Certification Program Participation in Yearly Basic Training  Surrogate Parent Code of Ethics Including Confidentiality Statement  Certified Surrogate Parent Information & Conflict of Interest Agreement LEA Contacts Appropriately Trained Surrogate Parent  Assignment on a rotating basis and based on factors including special skills of surrogate, where surrogate resides or works and demographic congruence  If contact accepts the assignment then additional information will be shared.
