ADMISSION POLICY Admission to the ICSB / ICSM Metropolis Junior & Senior High School Department & ICSM Main & Metropolis Pre-Elem. & Elementary Department is granted with the understanding that the parents/guardians agree to comply with the rules and regulations of the school with its scholastic standards for the child / student.

APPLICATION REQUIREMENTS Pre-Elementary–Grade 5 Photocopy of Birth & Baptismal Certificates (with Original Copy) Grade 7 & Transferees (Grs. 8&9)  Photocopy of Birth & Baptismal Certificates (with Original Copy) Photocopy of Report Card with grades up to the 3rd Grading. Letter of Recommendation / Good Moral Cert. from the Head of the previous school.

APPLICATION PROCEDURE Secure the Application Form from the Student Development Center (SDC). Submit the requirements. Return the accomplished Application Form to the Student Development Center (SDC). Pay the Testing Fee (Php 300.00) at the Cashier’s / Finance Office Proceed to the SDC and claim the Test Permit with the Test Schedule indicated in it.

AGE REQUIREMENTS (Age by June 1, 2015) Nursery at least 3 years old Kinder 1 & 2 at least 4 & 5 Years old Grade 1 at least 6 years old

TESTING PROCEDURE Present Testing Permit at the Student Development Center (SDC) on the specified testing date. Bring pencil with eraser. Additional crayons for Kinder 1&2 and Grade 1 applicants. Applicant takes the test independently. Applicant should report promptly at the SDC for their scheduled interview. Test results will be announced within ten (10 ) working days from testing date.

ENROLLMENT PROCEDURE REQUIREMENTS FOR ENROLLMENT: Photo copy of Birth Certificate (NSO Authenticated) Photo copy of Baptismal Certificate Original copy of Report Card (Form 138) Good Moral Certificate Test Result Form (issued by the Student Development Center) NAT Rating - Grade 6 (for incoming Grade 7) Health Certificate 2 copies 1x1 colored photo 1 long folder

ENROLLMENT PROCEDURE FOR NEW STUDENTS / TRANSFEREES: Submit all the Requirements at the Registration Officer. Secure the Registration Form from the Registration Officer Fill-out the Registration Form legibly and correctly. Pay the necessary fees ( PTC and CBS ) Proceed to the Registrar’s Office to secure the Clearance Slip for Enrollment Proceed to the Finance Office for payment. Pay and Pick-up the textbooks at the Book Custodian’s Office
