Purchasing Card Authorization Form  Completed form is submitted and approved prior to making purchases (accompanied with the Activity Report Form)  Copies of all invoices/receipts are submitted within 5 days of purchase  Included on form:  Name of person making the purchase  Last 4 digits of card number  Estimated cost of purchase  Authorizing signature(s)  Account numbers  Vendor & Purpose

Input from PARENTS  Parent Survey results – 1520 surveys returned TOPICS generating the most interest for parents:  #1 – Motivating children  #2 – Helping children with homework  #3 – Family fitness and health  #4 – Managing children’s behavior SERVICES Requested  #1 – After school program  #2 – Online tutoring or enrichment for home use  #3 – Homework Hotline  #4 – Parent Calendars

Input from PARENTS  Parent Survey  93.29% support more spending on Interventionists in Reading, Math, and Science  88.4% support After-School (Day Care) and Summer School Programs  82.38% support class size reduction  82.29% support spending for FREE books  81.57% support spending for summer programs
