Safety and Health Program Section B of the Forest Activities Code

Management commitment: Identify the safety and health personnel and resources that will be used to implement the program. Establish a policy that provides for ongoing evaluation of employees safety performance. Establish a disciplinary policy to address unsafe work practices. Authorize a competent person for each jobsite who has authority to : Supervise all personnel at the site. Enforce the companies safety and health policies.

What is the companies disciplinary policy on violating work practice rules? Who is assigned to be the competent person at your worksite? Where are copies of company safety policies located?

Supervisory responsibility: Supervise all employees at the site and enforce company policies. Verify that all current and new employees can perform assigned tasks and have received adequate instructions and training. Periodically review the safety performance of each employee. Provide training or discipline to an employee working in an unsafe manner. Closely supervise employees receiving instruction and training.

How does the supervisor assure compliance with company policies? How does the supervisor evaluate employees performing their duties?

All fatalities and catastrophes must be reported to OR- OSHA within 8 hours. All injuries requiring admittance to a hospital overnight with treatment must be reported to OR-OSHA within 24 hours. Employers or their representative must preserve the accident scene where their was a fatality or catastrophe.

Who is supposed to notify OR-OSHA? How can we preserve the accident scene?

Accident investigation: Investigate every fatal and recordable injury. Discuss near misses with employees. Inform employees of preventative measures developed from investigations. Keep written reports of investigations and corrective measures for 3 years.

Who does our accident investigations? How do employees report near misses? Where do we keep information on accident investigations? When do we discuss near misses?

Employee involvement- The employer or their representative must: Encourage employees to participate in site planning and pre- work safety meetings. Require employees to report hazards. Require qualified employees to correct hazards. Conduct monthly safety meetings with all employees. Keep written minutes and attendance records for 3 years. Make written records and attendance records available to all employees.

How do employees report hazards? Who takes corrective actions to eliminate hazards? Where are safety meetings held? How do they get information from the safety meetings to all employees? Where are the safety meeting minutes kept or posted?

Hazard identification: Implement a procedure for monthly safety inspections. Identify who will complete the inspections. Implement a procedure to report and correct hazardous conditions. Who does the monthly inspections? Who is in charge of assuring corrective actions are taken?

Training Provide safety and health training to current and new employees: Starting their initial work assignment, or Being assigned new work tasks, tools, equipment, machine or vehicles. Evaluate new employees who have been previously trained. Assure that a qualified person presents instruction and training.

Provide training that includes: Safe performance of assigned work. Procedures, practices and requirements of the work site. Recognition of hazards associated with each employees specific work tasks, including measures and practices to prevent and control those hazards. Safe use and maintenance of tools, equipment, machines and vehicles each employee uses or operates. Require each new employee receiving training to: Work under the close supervision of a qualified person. Demonstrate to the employer the ability to safely perform their work assignment before they work independently.

Keep a written record of instruction and training for each employee. Review the safety and health program annually. Who does the companies training? How do trainees demonstrate the ability to perform task? What are the qualifications of the trainer? Who reviews the safety and health program?
