A brief history… SCHC opened in the realization of a dream shared by six caring medical people early in their careers - actually three husband-wife teams: Julio Porro and Jennifer Hastings, Jeff Meckler and Maria Pacheco, Robert Azcarraga and Gina Ramirez-Azcarraga who saw the need for community-based medical center for people living in southwest Santa Rosa. With the help of Sister Michaela Rock and Santa Rosa Memorial’s Community Benefit Program, they developed a vision of a medical facility that would meet the needs of uninsured, primarily Latino community living in southwest Santa Rosa. Today the SCHC is an independent, federally qualified community health center with 5 locations. We have become true community asset, the fulfillment of a dream shared by people who listened to their hearts and persisted in their dreams. And, we have much more work to do!

Hours of Operation Monday – Thursday: 8:30 AM – 9:00 PM Friday: 8:30 AM – 5:00 PM Saturday: 9:00 AM – 3:00 PM Bilingual/bi-cultural providers and staff Services are provided on a sliding scale based on family income.

Who do we serve?  We serve the uninsured and underinsured, low- income families of Santa Rosa – 40,000 visits a year (150 visits per day) – 96% earn less than $25,000/yr for a family of four – 69% of the patients are Latino – 48% of the patients have NO insurance – Over $750,000 in uncompensated care each year

Services We Provide  Primary Care – Acute care – Well-child exams, including vaccines – Prenatal Care and Obstetrics – Family Planning – Mental Health counseling – Adult physicals, school physicals, sports physicals, work physicals

Services SCHC Provides  On-site Specialty Services – Podiatry – Chiropractic – Ophthalmology – Gastroenterology – General Surgery Consultation – High risk OB and Gynecology consultation  Referrals for: – Dental Services – Mental Health – Specialty Care

Services we provide  Health Education and Wellness – Diabetes education and case management – Parenting classes – Childbirth and breastfeeding classes – Nutrition classes – HIV/STD education – Reproductive health education

Services we provide  Outreach to the Community – HIV and STD outreach – Bus vouchers and taxi vouchers – Health fairs – Youth development program – Teen Pregnancy Prevention – MediCal and Healthy Families application assistance

Challenges for SCHC Changing demographics of our patients – Majority of patients are employed – More middle income, uninsured – Percentage of Latino patients is declining Growing number of uninsured due to: – Rising cost of premiums – catastrophic coverage only, employers opting out of offering health care benefits. Budget cuts to health care services Lack of access to: – Mental health services – Specialty care – Dental services Increases in serious illnesses and health disparities – Diabetes – Obesity – Teen pregnancy – Mental illness
