Risks are an integral part of life and, therefore, work. It is an unwelcome event within the Trust when a service user, visitor, or staff member encounters a risk which results in them being harmed in some way. The way these risks are managed is called Risk Assessment. Risk Management? Risks are an integral part of life and, therefore, work. Risk Assessment A Risk Assessment is: "a process which identifies, eliminates or controls hazardous situations" It should ensure a safe place of work. There are a variety of Risk Assessments (RA) which are used within the Trust. Here are just some of them: General Workplace RA Violence & Aggression RA COSHH RA Manual Handling RA DSE RA A Risk Assessment is a formal way of identifying what risks exist in the workplace and looking at how these risks can be eliminated, or, if they cannot be eliminated, how they can be reduced and controlled.

SEPT - MANDATORY TRAINING Why Risk Assess? The Health & Safety at Work etc Act 1974 implies the need for Risk Assessments to be carried out. The Management of Health & Safety at Work Regulations 1999 specifically require the use of Risk Assessment. This means that Risk Assessment must be carried out in the workplace, by law. Regulation 3 states: "Every employer shall make a suitable and sufficient assessment of (a) the risks to the health and safety of its employees to which they are exposed whilst they are at work; (b) the risks to the health and safety of persons not in their employment arising out of or in connection with the conduct by undertaking, for the purpose of identifying the measures they need to take to comply with the requirements and prohibitions imposed upon them" [...] Risk Assessments must be done, and they must be suitable and sufficient for what they do Definitions The terms most used in Risk Assessment are "Risk", "Hazard", and "Rating". An Hazard is: The potential of something to cause harm A Risk is: The likelihood that the identified hazard will cause harm For example: In an hallway the linoleum bubbles-up and cracks, giving a place where someone could catch their foot. This would be the hazard. The risk is how likely someone would be to actually catch their foot: rare, unlikely, possible, likely, almost certain. The Rating is: How severe the harm will be if someone or something is harmed or damaged Definitions The terms most used in Risk Assessment are "Risk", "Hazard", and "Rating". Process Risk Assessment is an easy process which can be broken-down into the following steps: Identify the hazard Identify who the hazard can affect Identify the risk Identify what is in place now to control risk and rate it What further controls are needed if the current controls are not sufficient, and then re-rate Monitor and review Each step must be written down, and the Trust Risk Assessment forms make ample provision for this. The old legal adage - if it is not written down, it did not happen - applies. SEPT - MANDATORY TRAINING Trust Risk Matrix The Trust uses a matrix to help people complete the Risk Assessments. The likelihood of an hazard, and the severity, are both identified with the two numbers being multiplied together to give the overall risk. This final number is identified on a Rating Table to give the overall Rating. For example: An hazard identified as "likely" to occur = 4 and a severity, if it does occur, is "minor" = 2. 4 x 2 = 8 Trust Risk Matrix The Trust uses a matrix to help people complete the Risk Assessments. Integrated Risk Health & Safety is only one part of Risk Management within the Trust. The Health & Safety Officers are themselves part of the Risk Management Department (RMD).
