Report # Deluge Gun Fails at Fire. Report Number: Report Date: 11/17/ :36 Synopsis Deluge gun fails at fire

Event Description Brackets [] denote reviewer de-identification. On a single-family residential working structure fire, the incident commander (IC) advised to prepare for change in the mode of operations. The fire apparatus operator (FAO) set the pre-piped deluge gun for operation. The IC advised to start the defensive operations as the FAO opened the valve and started flowing the master stream device. In less than 1 minute of operation, the deluge gun monitor came loose from the base (this is a permanent mount model). As a result, the monitor struck the FAO which resulted in minor scrapes and bruising. Minor damage also occurred to the piping and monitor. After investigation, it was found that the ball bearings used in the swivel operation had dropped out over time after the dust cap had popped out. The manufacturer [name deleted] had placed a snap-in dust cap instead of screw cap to seal the bearings in place. It was fixed by placing a sealant in the dust cap to help prevent the cap from falling out again. Lessons Learned Check all equipment on a regular basis, even the little things #
