Patient- and Family-Centered Care — What is it? Patient- and family- centered care is working with, rather than doing to or for.

Patients and Families: Bring important perspectives about the experience of care. Teach how systems really work. Inspire and energize staff. Keep staff honest and grounded in reality. Provide timely feedback and ideas. Are partners in Safety and Quality

Patient- and Family-Centered Principles People are treated with respect and dignity. Health care providers communicate and share complete and unbiased information with patients and families in ways that are affirming and useful.

Patient- and Family-Centered Principles Individuals and families build on their strengths through participation in experiences that enhance control and independence. Collaboration among patients, families, and providers occurs in policy and program development and professional education, as well as in the delivery of care.

Health care should be based on continuous healing relationships. Care should be individualized. It is important for patients to be involved in their own care decisions. Patients and families should have improved access to information. Health care should become more transparent. Institute of Medicine

Something to think about What Can I Do What Can I Do?

-Families can certainly assist with this Coler-Goldwater Specialty Hospital and Nursing Facility modified and expanded the Institute for Healthcare Improvement's inpatient ventilator bundle for use in the long-term acute care setting. Key elements of the modified bundle include early and ongoing evaluation for weaning, pulmonary and dental evaluations, head-of- bed elevation, peptic ulcer disease and deep vein thrombosis prophylaxis, and comprehensive oral care. Pre- and post-implementation analyses suggest that the program has reduced the incidence of ventilator-associated pneumonia by 58 percent, which, in turn, has led to a decline in the overall facility pneumonia rate and use of antibiotics. Families can certainly assist with this

The awards focus on central line infections and ventilator- associated pneumonia, two of the deadliest and most expensive hospital-acquired infections.
