Denise’s Story Venus is a 46-year-old woman participating in the ParTNers for Health Wellness Program for lifestyle management. Venus Venus’ achievements include: Exercising by walking 4 days per week. She has plans to wear extra warm clothes to maintain her routine during the winter She has lost 12 pounds since beginning the program. Making healthy changes to her diet by adding whole grains, vegetables, and using portion control Remaining tobacco free and feels very confident in her ability to stay quit Recommendations and support from the Health Coach: Track daily intake of foods Portion control Benefits of exercise Benefits of planning ahead “Thank you so much for the encouragement!” -- Venus

Jimmy Jimmy is a 52-year-old man participating in the ParTNers for Health Program and working on weight management and nutrition. Jimmy’s achievements include: Recovering from his procedure without complications and is now cancer free Loosing a total of 36 pounds and has a new goal to lose 30 more Exercising 3 days per week doing cardio and strength training Setting a new exercise goal to ride his bike 10 miles 2-3 days per week Recommendations and support from the Health Coach: Daily servings of fruits and vegetables Benefits of exercise Using motivation to achieve goals “The grandchildren are my motivation to be healthier. Deborah Deborah is a 59-year-old woman participating in the ParTNers for Health Program and working on weight management. Deborah’s achievements include: She has lost 12 pounds Decreased her total cholesterol from 215 to 169 Decreased her LDL(bad cholesterol) from 141 to 107 Increased her HDL(good cholesterol) from 38 to 42 Exercising by walking twice weekly Recommendations and support from the Health Coach: Eliminating high sugar foods/drinks from diet Increasing soluble fiber to improve cholesterol Benefits of exercise Overcoming barriers “I am motivated to keep working on my goals because I enjoy my clothes fitting better and I am excited about my success so far!”-- Deborah

Anonymous Member A 50-year-old female with multiple chronic medical conditions participating in the ParTNers for Health Program who told her coach she recently attempted suicide and continued to have suicidal thoughts. Member’s achievements include: Regularly seeing a psychiatrist Emotional health has improved through support of her family and her coach Has created a depression action plan Seeing a new primary care physician regularly to follow up on physical care Recommendations and support from the Health Coach: Immediately warm transferred her to a Magellan counselor to keep her safe from self harm Worked closely with her to monitor her emotional health and physical conditions During the following weeks there were frequent check-ins to provide continued guidance, education and support “I feel like a weight has been lifted from my shoulders. Your team has given me direction and support when I greatly needed it.” ParTNers for Health Wellness Program member

Partnership Promise

Partnership PPO Members and Covered Spouses Must: Complete the online Healthways Well-Being Assessment ® (WBA) by March 15, Actively participate in health coaching, if you are called. If you are called for health coaching, you must complete a biometric screening. Health coaching could include a tobacco cessation program and/or case management. Keep your contact information current with your employer. Covered spouses must keep contact information current with Healthways, if it changes Partnership Promise Requirements

Partnership Promise Requirements New Hires/Members: (as of 1/1/15) Who enroll in the Partnership PPO must complete the online Healthways Well-Being Assessment ® (WBA) and get a biometric screening within 120 days of their insurance coverage effective date Communication Strategy: January 2015 wellness program welcome mailer Notification of Partnership Promise completion throughout the year. Reminder s for WBA and biometric health screening
