Projected Outcomes Increase Clinic access –Increase overall discharges; particularly planned discharges –Decrease client length of stay –Change caseload composition Engage the client in discharge planning (client centered focus) Increase clinician awareness. Remain focused on treatment plan/discharge plan.

Moving Forward… New Documentation Requirements –Intake assessment –Treatment plan reviews Monitoring Progress –Compare quarterly planned/unplanned discharges with 2009 baseline data. Both overall and individually. –Provide clinicians data/feedback –Continue education

Impact on Clinic Culture Clinician focus on shorter term treatment via promotion of natural supports. Expectation of goal positive graduation from services. Staff creativity encouraged to move clients toward discharge.
