If the teacher refuses to sign, the administrator places “Teacher Refuses to Sign” in the comment section of that page with the phrase: “Witnessed by: Administrator Name (and one other person usually the confidential secretary or another administrator in that space)”. Once the principal electronically signs, the form will be sent to HR and electronically placed in their personnel file.
