To develop an Age Wave Readiness Toolkit to empower individuals to take ownership of their own aging and community engagement, as well as, provide a mechanism to build broader awareness of resources available across the region. This work includes: identifying evidenced-based or practice-based content, sources for content, methods of dissemination and product development.

 Regional information (inclusive of all PD15) Benefits the individual (elders and caregivers) and is person-centered.  At least 12 months in existence.  Contents without an expiration date.  Responds to a Community Need or Opportunity  Supports positive and optimal aging.  Vetted by the Committee (for relevance and appropriateness)  Evidence-based materials

 These resources support the 80/20 rule. 80% of elders are doing a FABULOUS job of aging. They are independent, engaged, active and loving life. The following resources can support increasing that number to 100%
