Introduction - 1 Risk Assessment and the use of a Risk Assessment Matrix (RAM) require a memory function. Risk assessment for low frequency, high potential incidents (Fatalities) needs to be done on the basis of collective industry experience. Need a repository for Industry experience. “Aide Memoir for Geophysical Risk Assessors” Proposed to April 2006 HSE forum and agreed action. Work in progress, further contributions invited!

Introduction - 2 Prime objective – Tutorial for next generation (“mandatory reading material”) Avoid repeats and re-inventing the wheel the hard way ! – Reference database for use with RAM, capturing industry experience Answer the question “Has this happened in the industry ?” Side benefits – Improve statistical insight – Identify “Rare” but real risks Further use – Cross check when developing guidelines, e.g. OGP 317 and IAGC HSE manuals Have we covered all known risks that have resulted in a fatality in the past?

Data base content and structure Excel spreadsheet – Sort and Search functionality – Graphical analysis tools Fields used: – Record number for reference (invariant to sorting) – Year – Number of fatalities in event – Single or Multiple (>3) fatality – Summary accident description – Comments, identified root causes if available – Geophysical or Other operation – Land / TZ / Marine – Type of accident, broad standard classification – Type of accident, more detailed classification – 3 Keyword fields

Data base content and structure Not included – High potential Near Misses – Common vehicle accidents Limit size and effort, but may be we should add these too ? – Country (exception: bee attacks) – Company or Contractor information – Information about victims, such as gender, age, time employed etc.

Data base sources OGP annual reports since 1980 IAGC and CAGC data and HSE alerts STEP change Sadie database alerts WesternGeco CGG Many experienced individuals – Reports “from memory” accepted from reliable sources – All fatal accidents in Geophysics that we could find (except vehicle accidents) – Relevant accidents from other E&P operations that “could easily have happened in Geophysical Operations”

Data base So far captured: – Geophysical operations: 379 fatalities from 219 accidents – Other 217 fatalities from 181 accidents Please contribute and send info on further fatal incidents, not yet in the data base, to E. Sixma Plan to “package” data base with some introductory and explanatory text and to publish as OGP report towards the end of Excel data base will also remain available through OGP / IAGC websites. Update database yearly
