M/V KALAKALA What happened: – Vessel listed 20° to port – Water intruded into the bilge – 100 gallons of motor oil in the engine Significance: – Threat of sinking or capsizing – No power to the bilge pumps – OSLTF opened

M/V HYUNDAI OAKLAND What happened: – Hazmat release of Parachlorobenzotrifluoride from a container – 2 people received chemical burns – Cause was unsatisfactory blocking and bracing Significance: – Release of hazardous material – Personnel injured – Unsatisfactory blocking and bracing found in 6 containers

P/C ANDORINA What happened: – Vessel grounded and sank as the tide came in – Vessel started to discharge – OSLTF opened for dive ops and fuel removal Significance: – Submerged for duration of response – 150 gal potential – Owner financially unable to pay for response – OSLTF opened

F/V RUBY LILY What happened: – Vessel grounded – OSLTF opened, but insurance agency took responsibility – Vessel had 30° list during defueling, ops halted – Vessel refloated with the tide Significance: – 4800 gallon potential spill on a State Park – 4 contractors hired with 5 vessels on scene – Ballast concerns during defueling
